Job Description

1. Medication Management & Patient Safety


a. Prescription Review

  • Verify Legibility, Authenticity, Completeness of prescriptions.
  • Check for right drug, right dose, right route, right patient, right duration.
  • Identify and prevent drug interactions, duplications, allergies, contraindications.

b. Dispensing

  • Dispense medications accurately following double-check protocols.
  • Follow LASA (Look-Alike Sound-Alike) precautions.
  • Ensure dispensing according to FIFO/FEFO.

c. Patient Counselling

  • Provide counselling for dose, frequency, storage, missed dose, and side effects.
  • Educate patients especially for high-alert medicines, anticoagulants, insulin, inhalers.

2. Medication Storage, Distribution & Inventory Management


a. Procurement & Inventory

  • Maintain optimal inventory using ABCVED analysis, consumption trends, buffer stock.
  • Ensure availability of life-saving drugs, emergency drugs, antidotes.

b. Storage Requirements

  • Maintain appropriate temperature, humidity, documentation for drug storage.
  • Ensure segregation of:
    • High-alert drugs
    • Narcotics & controlled substances
    • Near-expiry and non-moving items
    • Returned or recalled drugs

c. Controlled Substances

  • Maintain NDPS register, usage logs, daily counts, and secure storage.
  • Comply with NABH requirements for restricted drugs.

3. Compliance with NABH Standards


a. Documentation

  • Maintain:
    • Pharmacy SOPs
    • Temperature logs
    • Recall registers
    • Expiry registers
    • Calibration records
    • Indent tracking
    • Drug formulary updates

b. Audits and NABH Assessments

  • Participate in internal/external audits.
  • Present evidence for MOM (Medication Management), COP, HIC, and QMS compliance.

4. Medication Safety & Quality Assurance

  • Participate in medication error reporting (near miss, no-harm events, adverse drug events).
  • Implement corrective and preventive actions (CAPA).
  • Participate in Medication Safety Committee.

High-Alert Medication Management

  • Enforce special protocols for:
    • Insulin
    • Heparin
    • Chemotherapy drugs
    • Electrolytes
    • Narcotics

Participate in ward visits with medical teams.

  • Monitor medication therapy, adverse drug reactions, and compliance.
  • Maintain ADR reports and inform the necessary at Hospital
  • Support therapeutic drug monitoring (TDM) where applicable.

6. Hospital Formulary and Antibiotic Stewardship


a. Formulary Management

  • Maintain and periodically update the Hospital Formulary.
  • Monitor non-formulary drug indenting and rational usage.

b. Antimicrobial Stewardship (AMR/AMS Program)

  • Ensure availability of appropriate antibiotics.
  • Monitor usage of restricted antibiotics.
  • Participate in AMS rounds and prescription audits.

7. Emergency & Critical Care Support

  • Ensure continuous supply of crash cart drugs, ICU medications, OT drugs.
  • Perform routine crash cart checks and documentation.
  • Refill emergency kits and maintain sterility standards.

8. Training & Capacity Building

  • Train nurses and staff on:
    • Safe medication administration
    • LASA precautions
    • Antibiotic stewardship
    • High-alert drug management

9. Pharmacy Billing & Ethical Compliance

  • Ensure correct billing, coding, and transparency.
  • Avoid irrational drug promotion or unethical practices.
  • Ensure compliance with:
    • DPCO
    • Pharmacy Act
    • Drug & Cosmetic Act

10. Participation in Quality Improvement Programs

  • Participate in:
    • Quality Indicator data collection (e.g., medication errors/1000 prescriptions)
    • Root cause analyses
    • Patient satisfaction surveys

Qualifications & Skills:


  • Education: Diploma or Degree in Pharmacy.
  • Experience: Proven experience and understanding of hospital pharmacy services for 4+ years.
  • Communication Skills: Strong interpersonal skills to engage with customers effectively.
  • CRM Proficiency: Familiarity with Hospital Pharmacy HIS tools.

  • Candidate should possess exceptional communication skills Written [English + Kannada] and Verbal [ Kannada + English + Hindi + Tamil + Telugu].
