- Coordinate the admission process for in-patients, ensuring all necessary documentation is completed.
- Serve as the primary point of contact for patients and their families, providing updates on treatment and addressing any concerns.
- Collaborate with medical and nursing staff to develop and implement individualized patient care plans.
- Monitor patient progress and assist with discharge planning, ensuring that patients transition smoothly to the next phase of care.
- Maintain accurate patient records and documentation, adhering to hospital policies and regulatory requirements.
- Participate in quality improvement initiatives to enhance patient satisfaction and service delivery.
- Provide information to patients regarding hospital services, policies, and procedures.
- Creating wow-customer expects.
- Organising Patient Pulse Meetings with key-stakeholders for suggestions and improvements.
- Organise Home visits.
- Actualise Patient need Celebrations. Skills and Tools Required:
- Excellent communication and interpersonal skills to interact effectively with patients, families, and staff.
- Strong organizational and multitasking abilities to manage multiple patient cases simultaneously.
- Proficiency in using hospital management software and electronic health records (EHR) systems.
- Problem-solving skills to address patient concerns and facilitate timely resolutions.
- A compassionate and empathetic approach to patient care.
- Knowledge of healthcare regulations and hospital policies.
- Ability to work collaboratively in a fast-paced environment. Education and Experience:
- Bachelors degree in healthcare administration, nursing, or a related field.
- Previous experience in a healthcare setting, particularly in patient coordination or care management, is preferred for a minimum of 5 years.
