Job Description

Clinical Nutrition & Assessment

• Conduct detailed nutritional assessments of patients based on medical history, clinical condition, dietary habits, and lifestyle.
  
• Identify specific nutritional needs for individuals with chronic or acute health conditions (e.g., diabetes, cardiovascular issues, gastrointestinal disorders).
  
• Provide medical nutrition therapy (MNT) to help manage disease processes and promote recovery. Apollo Home Care
  

Personalized Meal Planning

• Develop customized diet plans tailored to each patients medical condition, preferences, and cultural/food habits.
  
• Adjust diet plans based on ongoing patient progress and changes in health status. Apollo Home Care
  

Telehealth & Remote Support

• Offer tele-dietetic consultations and follow-ups via phone/video calls to support patients who receive home care remotely.
  
• Educate patients and caregivers on healthy eating practices, meal preparation, portion control, and lifestyle modifications. Apollo Home Care
  

Collaboration & Teamwork

• Work closely with nurses, physicians, and other allied health professionals who deliver care in the home environment to ensure integrated care planning.
  
• Communicate diet plans and recommendations with the multidisciplinary clinical team. Apollo Home Care
  

Documentation & Reporting

• Maintain accurate clinical records documenting assessments, diet plans, progress notes, and patient education provided.
  
• Ensure compliance with organizational quality and clinical safety standards.
