Job Description

1. Conduct patient assessments to determine functional limitations and therapy needs.

2. Develop individualized treatment plans based on patient evaluations and goals.

3. Implement and monitor therapeutic interventions, adjusting plans as necessary.

4. Assist patients in developing daily living skills and improving their ability to perform daily tasks.

5. Maintain accurate records of patient progress and treatment outcomes.
