Roles and Responsibilities:

Assist with daily activities such bathing, feeding and dressing patients.
Monitor changes in behavior and progress.
Provide hands on care to patients at home.
Do daily assessments and documents finding in the nurses notes in a timely manner.
Willingness to work for a home care environment.
