Provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Requires a bachelor's degree in Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. Gaining exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Typically requires 2 to 4 years of related experience. Develops interdisciplinary care plan and other case management tools by participating in meetings; coordinating information and care requirements with other care providers; resolving issues that could affect smooth care progression; fostering peer support; providing education to others regarding the case management process.

Eligibility Criteria-

1. Age: 21-49 yrs.

2. Qualification: Candidate should be a graduate/diploma holder in the relevant field.

3. Experience- Minimum 2 years of experience in the relevant field.

4. Good communication skills (mandatory)

5. Clients should have sufficient funds to sustain in the country. Eg: Real Estate Property, Bank Balance Bank Deposits, Fd, PPF.

IELTS not mandatory in Hongkong

Visa duration- 7 years
